Since 1993, Root for Kids has provided over 16,000 families with the extra help they need to navigate challenges and ensure that every child in our community has the skills they need to excel in school and in life. Occupational Therapist
REPORTS TO: EARLY INTERVENTION DIRECTOR
Summary: Provide instruction and information to parents in a home-visit model as prescribed by
the Individual Family Service Plan. Conduct standardized, criterion referenced and observational
assessments to determine eligibility and functioning of referred children. Coordinate and monitor
the timely delivery of identified evidence-based Occupational Therapy services. Collaborate and
team with IFSP team members, including the family, to develop Individual Family Services Plans.
General Functions:
- Conduct standardized, criterion referenced and observational assessments to determine
eligibility and functioning of the child in the areas of sensory processing, oral motor and
feeding development. - Explain family rights and procedural safeguards to families in understandable terms,
including when and how they may be applied, to support them in advocating for their
child and family. - Coordinate evaluations and assessments, identify and facilitate the multi-disciplinary
eligibility determination process, and ensure family participation through this process. - Explain developmental delays and disabilities and their impact on children’s development
and learning to the family to support intervention decisions. Explain how each domain
contributes to the whole child, and how ongoing progress is monitored. - Address the priorities and concerns of the family and support them by developing
appropriate child and family goals. - Orient all families to the EI system and discuss formal and informal supports, and
opportunities available to them. - Explain to families how EI is funded and answer any questions regarding fees and
payments when appropriate. - Respect and support the distinctive qualities of each family, recognizing the family as the
teacher, support, decision-maker and advocate for the child. - Build professional, supportive partnerships with families.
- Work with families and other adults to identify each child’s needs for assistive technology
and acquire or create appropriate assistive technology to promote access, participation
and learning within and across activities and routines. - Provide needed training to EI Staff in the area of Occupational Therapy and provide
supervision to Occupational Therapy Interns as needed. - Support a variety of planned and timely strategies for successful adjustment and positive
outcomes for both the child and family as they exit early intervention. - Use effective teaming and collaborations practices in order to support families and other
professionals in carrying out the IFSP.in a timely manner. - Support each child’s development and learning within natural environments.
- Assist families to access available community resources/services, parent groups, and
state/local advocacy organizations. - Ensure smooth transitions into and out of the program. Help families to understand
resources available to them as they exit the EI program. - Adhere to the principles of the DEC Codes of Ethics. Set personal and professional
boundaries and adhere to honesty, confidentiality, flexibility, resiliency, time management
and dependability. - Engage in ongoing professional development.
- Special projects and other duties as assigned.
Qualifications:
- Utah license in Occupational Therapy required. License in Arizona a plus.
- Must obtain a current Baby Watch Early Intervention Credential as mandated by the
State of Utah Department of Health within 6 months of hire. - Strong knowledge of infant/toddler development including factors that contribute to
development. - Excellent communication skills, both oral and written. Bilingual English/Spanish a plus.
- Ability to work independently and accurately perform job functions.
- Good computer/data entry skills.
- Must pass a background check with fingerprints and E-Verify.
Physical Demands:
- Adaptable to varying home environments including some with animals, pests, with or
without heat or air conditioning, varying levels of cleanliness, as well as different floor
coverings and seating arrangements. - Ability to lift 20 lbs, to sit, stand, stoop, bend, walk, talk, hear, use hands to grip, reach
with hands and arms, walk on uneven surfaces, and climb stairs with ease. Reasonable

essential functions of the job. - Must complete a health appraisal and a tuberculosis test. Root for Kids strongly
recommends and will pay for staff to obtain these immunizations: Tdap, Covid, Hepatitis
A and B. - Recognize symptoms of stress, anxiety, depression or other mental health needs in
yourself or the families you serve. Seek self-care or professional care resources as
needed.
Work Environment:
Will work out of the main office. Needed technology and supplies are provided. Must accurately
keep personal time sheet and mileage updated. This position requires travel for home visits,
events, or meetings/ trainings. Must maintain a valid driver’s license and have reliable
transportation. Schedule is somewhat flexible around the needs your team, the duties of your
position, and needs of the agency.

Bilingual (English/Spanish) Clinical Mental Health Therapist
Part-Time Position
Wage rate: starts at $25
Root for Kids mission is to nurture strong beginnings in all children, We are seeking a person who wants to make a difference for children ages birth to three and their families in our community.
Part Time position.

This individual will:
- Conduct standardized assessments of children ages birth to three and their families that have been referred to determine eligibility.
- Will develop treatment plans as indicated by assessments and assist in the development of Individualized Family Service Plans
(IFSPs) for children with special needs. - Therapy will be provided in both a home visit and center-based model.
- Will manage all referrals and assist with determining eligibility.
- Will provide training and consultation to staff, and document all services provided in a data base.
- Will also assist with the development of policies and procedures to address mental health concerns agency-wide.
Requirements include:
- Requires at a minimum a Licensed Clinical Social Worker (LCSW) license or equivalent for the state of Utah and experience working with young children.
- Must have effective communication skills.
- Bilingual Spanish/English needed.
- Must have good record keeping ability and good computer skills including data entry and reports.
- Requires a person with full mobility, the ability to see, hear and speak, and lift up to 20 lbs. Accommodations can be made as necessary.

- Requires a current driver license and reliable transportation to conduct home visits with employer paid mileage.
